Bridging Horizons brings together individuals, families, community partners, and the professionals who provide Home and Community-Based Services (HCBS) across California. Our purpose is simple: to improve the lives of people with disabilities by strengthening the services and supports they rely on. We create meaningful spaces where people can learn, connect, and collaborate so everyone—individuals and providers alike—feels informed, empowered, and supported.
At the heart of our mission is the annual Bridging Horizons Conference—a statewide gathering of individuals, HCBS professionals, advocates, agencies, and leaders. It’s our largest platform for education and impact, offering keynote speakers, interactive workshops, compliance updates, lived-experience panels, and opportunities to share best practices. It’s where innovation meets community, and where both individuals and providers gain tools that help elevate the quality of support across California.
Beyond the conference, we work with our local regional center, Central Valley Regional, and community providers to offer HCBS workshops, community events, and training opportunities throughout the year. These efforts help service providers stay informed, supported, and connected to a strong network across the state.
We serve as a bridge between organizations, leaders, individuals, and service providers—making sure essential information, resources, and opportunities reach every community. Through inclusive spaces, collaborative learning, and a shared commitment to person-centered care, we help build a California where every person with a disability is empowered to live a fulfilling, self-directed life.
